  • 1.  Referral form

    A challenge we often face is that the requested services often are inconsistent with the referral diagnosis/indication. For example, MFM consultation is requested for a prior abnormal ultrasound but the referring provider does not checkoff ultrasound, or genetic counseling is appropriate but not requested on the referral. 

    A team member proposed the following solution.  Share with the referring practices a list of common indications and the recommended services for this indication in the form of document titled "Maternal-Fetal Medicine Recommended Services".  On the referral form for requested services, we can have a box stating "MFM services as detailed on the "Maternal-Fetal Medicine Recommended Services form".  Below this on the form we can list the various individual services in the event that the referring provider does not want any of the particular services.

    Something like this would really help us too! It's a constant struggle, and time consuming to work with referring providers on getting the orders right.

    If you develop an MFM Recommended Services Form, would you be willing to share?

    This has been a problem for MFM for decades. We found that you need a referral form that you give to each practice . The major thing on it is Ultrasound with consult if necessary or consult with ultrasound if necessary. That covers genetics and most all problems . Also remember you can do a consult or UTZ and get permission the next day. Once we started this we did not need much more on the sheet maybe BBP or NST . Dan

    Hi Robert,
    In your referral form (which can also be adapted to your EMR), you can add a statement stating that additional ultrasound studies or testing may be done when clinically indicated unless specifically declined by the referring provider. This will help stay compliant. I am attaching our form which we have adapted to our EMR, though we rarely use it anymore.
