  • 1.  Fever in Labor and COVID testing

    Hi,
    We are trying to balance fever in labor and appropriate PUI/COVID testing.  How many COVID positives have people seen in women who had no symptoms on admission to L&D but then had fever during labor?  Should we test all these moms or only if atypical for Chorio and/or any respiratory sx?  Would you use temp of 100.4F or 100.0F as "fever" in labor for COVID evaluation?  (About 10% of our moms pre-COVID era had temp of 100.0 or higher during labor but about 3-4% had T >100.4.)  At this time we do not have rapid testing (Still 12-24 hr TAT).  I am aware of the published cases from Columbia and heard rumors of a few others but I haven't seen much published and no "denominator." Our positives so far have been moms who had sx prior to L&D.

    We started testing only atypical two weeks ago, but changed last week to all who develop fever as we enter peak in MA. Out of 12 or so tests none have come back positive and most were classic chorio. Reassuring, but still would advocate for testing given fever is one of the more common signs and implications on L&D and postpartum.
