  • 1.  Discontinuation of Transmission-Based Precautions in Patients with COVID-19

    Are there any existing recommendations for documenting clearance of COVID-19 in pregnant patients regarding discontinuing transmission-based precautions when patients come into L&D? In other words, if a patient had a documented mild COVID-19 infection a month ago and is clinically well, are centers following the CDC testing guidance?:

    Test-based strategy.
    • Resolution of fever without the use of fever-reducing medications and
    • Improvement in respiratory symptoms (e.g., cough, shortness of breath), and
    • Negative results of an FDA Emergency Use Authorized COVID-19 molecular assay for detection of SARS-CoV-2 RNA from at least two consecutive nasopharyngeal swab specimens collected ≥24 hours apart (total of two negative specimens)

    If patients have two negative NP swabs, are they "cleared" and do not need to follow special precautions for labor, delivery, and newborn care? With the concerns that have been raised for fecal shedding, are any centers adding anal swab specimens to their clearance protocol? Any advice or guidance would be most appreciated. Thanks!
