  • 1.  Treatment for moderate and severe covid19 pregnant patients

    As new information is evolving, would like to know what others are using or recommending 1st line treatment for moderate and severe pregnant covid19 patients.  Seems most are using hydroxychloroquine with or without Azithromycin for moderate and Remdesivir for severe.  Would appreciate other's input.  Thanks.

    We looked closely at our first 16 pregnant Covid-19 positive patients, Only two of them ( 23 and 24 weeks GA) both with Class II and III BMIs needed supplemental  nasal cannula O2 for a brief period,  to return to FiO2 over 96%. Both showed bilateral opalescent lung bases. They were both placed on hydroxychloroquine, azithromycin and steroids by ID consultants and hospitalized for 7 days. Both had markedly elevated CRP.  WBCs and platelet count WNL. Went home undelivered  Both remain stable.  The rest of 14 patients tested Covid-19 positive, were delivered with no complications and no therapeutic agents.  Our concern is whether we are seeing a distinct viral clade that may allow us to be so conservative.  We are very interested in knowing of your clinical findings and your outcomes.  Remember NIH opined neither for nor against HC, azithromycin and remdesivir,   but neither ACOG nor SMFM were in the panel that issued that guideline.  . ​
