What are you all doing in cases of use of antenatal corticosteroids?
Do you limit to cases > 34 weeks or use up to < 37 weeks?
Do you use any rescue course?

<34 weeks at Brigham and Women's Hospital. After 34 weeks at the community hospital only if someone has a scheduled procedure and no symptoms and only after a discussion with MFM.

I thought the concern for steroids in COVID has been dismissed and are now used to treat patients in inflammatory phase. So, would return to regular steroid indications.

Phil, do you have any references regarding steroid use in the general COVID positive population? Or the COVID positive pregnant population?

    Dear Caroline,
I heard steroids mentioned on JAMA podcast from Michigan from their ID person and another one from Mayo, but that was 2 weeks ago.  Since then have heard in recent podcasts avoiding steroids again due to an association with prolonged viral shedding.  Hard to keep up.  We are using antenatal steroids to 34 weeks which I think is last SMFM recommendation. Full time job reading the latest literature.  Can't wait from some real data.

    Dear Phil,

Thank you for your response! I'm on the same boat as you, private practice while trying to keep abreast.
We're currently following the AJOG MFM Labor and Delivery Guidance for COVID19 by Boelig et al which indicated steroids until 34 0/7 weeks for asymptomatic or mildly symptomatic COVID positive patients. There's an interesting article by Johnson & Gottlieb on Emergency Medicine News April 2020, "What's working for COVID19 patients" indicating steroids might help with ARDS (Lancet Respir Med. 2020. doi: 10.1016/S2213-2600[20]30079-5).
