I concur to use the O34.219 code for the repeat cesarean delivery (rather than the Z code; whenever there is an applicable O code, it is recommended to use the O code rather than the Z code). For the PP visit, you may use the Z code since she is no longer pregnant technically.

I would use O34.211 (Maternal care for low transverse scar from previous cesarean delivery). You could use Z98.891 (History of uterine scar from previous surgery) but the O code is more specific &/or appropriate for billing.
