It is usually better to have separate requests when possible, particularly if co-management or care transfer are desired. For a simple one-time consultation, stating "ultrasound, with consult if applicable" is acceptable.

After being in a MFM physician management role for the last 12 years, I have recently stepped back into an administrative role over perinatal services including the perinatal outpatient center. I have a physician wanting to put "Ultrasound with consult if applicable" as the only option for all ultrasounds and consults without designating if outpatient management is desired or not. This was unacceptable a few years ago but I don't know what the current requirements are. Does anyone know where the criteria and suggested wording for referral forms might be or could someone update me? I'm looking forward to returning to the management meetings. They always provided a wealth of information. Thank you for the help!
